I have had great luck with PCB Fab Express. I have used them mostly for small/prototype quantities but also for production quantities. They have delivered on time. My only complaint is they do not support Target files.
I have used PCB Fab Express for many years, 2-6 layers. They have lots of options that let you balance turn time, quantity, and features, with 5pcs/5 days usually the "sweet spot." If you need "quick and dirty" they offer a 2-layer no-mask in 2 days. They have always been very responsive to questions and to help resolve technical issues with the order.
